I hereby grant permission to Evergreen Youth and Family Services, Inc. and/or its representatives, to take and use photographs and/or digital images of me for use in news releases, advertisement, educational materials, and other promotional material. These materials may include printed or electronic publications, websites, or other electronic communications. I further authorize the use of these images without compensation to me. All prints and digital reproductions shall be the property of Evergreen Youth and Family Services, Inc. I am aware and understand that these images may be publicized during any period, and I authorize permission to share these images with or without prior approval of myself. I further agree that Evergreen Youth and Family Services is the sole owner of any media that fits the description above.
